How Can I Participate?

        If you use a Macintosh computer, we apologize that the online form will not function properly for you.  Please use another method of participation, explained below.

You have a choice of four ways to participate in this survey:

2. On a computer
3. On paper
4. Over the telephone

1. Online on the Internet

    You can do the survey on the Internet. The online Survey form works ONLY on a personal computer (PC) using ONLY Microsoft Internet Explorer, Version 5.0 or higher. You may have this software already installed on your computer. Please Click here to start survey now

2. On a computer

    You can download and do the survey as a Word document on a computer or, if you use a screen reader, we recommend you use the text version.
