Introduction

        Thank you for thinking about being involved in this study. It is being done by the Rehabilitation Engineering Research Center (RERC) on Accessible Medical Instrumentation. The study is being led by June Issacson Kailes, Associate Director of Western University's Center for Disability issues and the Health Professionas, in Pamona, CA and Dr. Jill Winters, from Marquette University's College of Nursing in Milwaukee, WI.

How Can I Participate?

        The purpose of the survey you are filling out is to help us learn about difficulties that some people with disabilities have using some medical equipment, such as devices used for diagnosing and treating medical problems.  If you finish part or all of this survey, it means you understand this study. Filling out this survey is voluntary, and you can stop answering questions at any time, without penalty.  Your identity will not be shared with anyone, and  your answers will not be linked to your name in any way.  This site is password protected, so no one other than the researchers will have access to your information.  Your name will not appear anywhere on the data we collect.  About 300 people with disabilities will fill out the survey. 

        If you use a Macintosh computer, we apologize that the online form will not function properly for you.  Please use another method of participation, explained below.

You have a choice of four ways to participate in this survey:

  4. Over the telephone

1. Online on the Internet

    You can do the survey on the Internet. The online Survey form works ONLY on a personal computer (PC) using ONLY Microsoft Internet Explorer, Version 5.0 or higher. You may have this software already installed on your computer.

2. On a computer

    You can download and do the survey as a Word document on a computer or, if you use a screen reader, we recommend you use the text version.
